Annual Recertification

If you have service under an TX Mobile Lifeline plan, we would like to say thank you for allowing us to serve you.

Each year, you are required to recertify that you remain eligible to receive discounted phone service through the Lifeline program. By recertifying your continued participation in one of the qualifying programs your service with us will be extended for an additional year. There will be no change or additional charges to your current account for recertifying.

Prior to the expiration date of your eligibility, You will receive a text, phone message, or a letter informing you that you must recertify your continued eligibility in the Lifeline Program. If you fail to recertify within thirty (30) days after being notified, or if TX Mobile is unable to verify your eligibility thru a state eligibility database, you will be deemed ineligible to continue participating in the Lifeline program. Lifeline discounts you currently receive will be discontinued and only Non-Lifeline discounted minute plans will be available for you to purchase.

If you are recertifying under a different program from that which you originally certified under, please provide a photo copy of proof of participation. Examples of acceptable forms of proof include copies of your EBT food stamp card, Medicaid Card, a current award letter you have recently received in the mail, or a letter from your case worker. Please do not send original letters or actual ID cards, as they will not be returned to you.

If you have moved from the address you originally provided on the initial enrollment form please contact us, within thirty (30) days, at the appropriate number below.  You can also contact us if you need to verify the address we have on file for you.

Please use one of the following options for returning your recertification:

By Mail.

Mail your completed recertification form to: TX Mobile, 9918 Brooklet Dr., Houston TX 77099

By Fax.

Fax the completed recertification form along with your proof of government assistance (if required) to 1-281-741-5540, Attention: TX Mobile Recertification Department. Include your name, a contact number, and your cell number including area code.

By Email.

You may also scan your completed recertification form and proof of government assistance (if required), and then email it to [email protected].
